Review of Medical Records:
- the patient's name on the passport (first, middle, last)
- patient's gender
- patient's address
- patient's date of birth
- direct patient phone number
- direct patient fax number
- patient's e-mail address, if available
- a brief explanation of the current medical problem (in English)
- the preferred date of appointment (admission)
- whether or not the patient will require an interpreter, and if so, what language
- E 112 from your health insurance company, if you are a member of the european union
A review of your medical history, including heart catheterization, by our specialists is necessary for determination of your admission to the MediClin Heart Center Lahr/Baden. This allows for an exact definition of your disease and of the necessary therapy. Please submit your medical records along with any pertinent specific test results, confidentiality is guaranteed.
Cost Estimate / Appointments:
As soon as your medical records have been reviewed and your diagnosis and the adequate treatment have been established, you will be advised of your admission and the approximate duration of hospitalization, you will also be informed of the intended treatment plan. Enclosed you will also find a list of hospital fees covering the complete spectrum of services rendered by our facility. Prior to your admission to our MediClin Heart Center Lahr/Baden we request an advance deposit to a specified account of the MediClin Heart Center Lahr/Baden or an advance cash settlement of the estimated costs upon arrival. We will offer you several admission dates. Please confirm the date suitable to you to enable us to prepare your stay at our facility.
